B.well Connected Health, which seeks to "simplify an overly complex health care delivery system," has closed a $16 million Series A investment round led by UnityPoint Health Ventures.

B.well's platform tracks health data and connects to doctors, insurers and wearable devices.

Other investors include ThedaCare and Walgreens Boots Alliance Inc. unit Well Ventures.

Previously, b.well raised $11.5 million in seed funding.
