Improving Transitions of Care for High-Risk Patients
In partnership with one of the largest community-based health
systems in Florida, Perficient will present “Improving Transitions
of Care for High-Risk Patients.” During this presentation, the team
will showcase how, through the use of IBM Watson technology, the
healthcare system innovated with natural language processing to
manage its Hospital Readmissions Risk and address MS-DRG bundled
payments.
“Eighty percent of health data is virtually invisible in current
health information systems and absent from data analysis, because
it is stored as blobs of text in an EMR or in a diagnostic report,
and cannot be easily mined to drive meaningful insights and
learning,” said Juliet Silver, Perficient’s director of health
sciences strategy. “During our session, you will hear how we
unlocked this valuable clinical data to inform transitions in care
for high-risk patients.”
In addition, the team will discuss its journey through a
successful proof-of-technology to reduce COPD readmission risk,
specifically, and its plans to deploy a similar approach to address
additional patient conditions. The presentation will take place on
Thursday, March 8 from 10:30 to 11 a.m. PST in IBM Booth #6243.
Cloud-Based Data Solution Improves Patient Care
Utilizing hybrid cloud technology to access data in real-time,
one of the largest non-profit healthcare providers in the United
States is making strides to reduce healthcare costs while improving

Kouba will present, “Cloud-Based Data Solution Improves Patient
Care,” in which he will explore how Perficient’s client journeyed
