BayCare Teams Up With Cerner, Lumeris and Salesforce to Improve Health of Senior Population

BayCare Health System, a leading not-for-profit health care system
in central Florida, is leveraging advanced technologies and
services from Cerner, Lumeris and Salesforce to better manage its
populations. BayCare’s overall strategy to increase access to care
and improve the health status of its patients is supported by a
combination of industry-leading solutions from these three
organizations. Clinicians are using population health strategies
and additional capabilities to extend care beyond the exam room and
improve health outcomes, inclusive of the health system’s senior
patients participating in its Medicare Advantage plan.
Foundational to BayCare’s approach is leveraging Cerner’s

expertise. BayCare is collaborating with all three to identify and

BayCare is using Cerner’s big data and insights platform for
population health management to analyze medical data across
platforms and support clinicians in achieving actionable insights
to help engage people beyond the hospital setting and manage health
outcomes. A single view of an individual’s health experiences, by
aggregating clinical data in near-real time from multiple sources,
supports BayCare clinicians with decision support to guide
treatment and care. BayCare’s population health management
approach, supported by Cerner, emphasizes providing high-quality
and proactive care to engage patients and improve care.

engagement and care plan adherence goals.

better outcomes for seniors enrolled in the BayCarePlus Medicare
Advantage (MA) health plan through their strategic operating
