( 1)	St. Jude Medical, Inc. restricted stock unit award that was converted to an Abbott restricted stock unit award in connection with Abbott's acquisition of St. Jude Medical, in a transaction exempt from Section 16 under Rule 16b-3. The restricted stock units are vested and will settle in shares on July 4, 2017.
( 2)	Held in the Alynne Starks 2012 Irrevocable Trust. The reporting person is the sole trustee of the trust.
( 3)	St. Jude Medical stock option that was converted to an Abbott stock option in connection with Abbott's acquisition of St. Jude Medical, in a transaction exempt from Section 16 under Rule 16b-3.
