Item 1.04. Mine SafetyReporting of Shutdowns and Patterns of Violations
On June 6, 2019, one of the mining facilities in the soda ash business of Genesis Energy, L.P. (we, or our)
received an imminent danger citation issued by the Mine Safety and Health Administration (MSHA) under section 107(a) of the Federal Mine Safety and Health Act of 1977. The citation was issued at the Westvaco facility near Green
River, Wyoming, upon observation of a surface maintenance employee standing on the blade edge of a road scraper while the equipment was being operated. The activity was immediately stopped and the employee standing on the blade was issued an
imminent danger citation by MSHA. No injuries or operational interruptions occurred as a result of the cited condition. We are investigating the matter further and intend to take all necessary corrective action including additional training of
our employees. The safety of our employees is our primary concern.
