received Patient Oriented Accreditation from the National Committee
for Quality Assurance (NCQA) for its Disease Management programs

recent Accreditation represents 15 consecutive years of
Accreditation for Health Dialog’s Disease Management programs,

award extends the Accreditation period until October 27, 2020.
