(1)	The number of options associated with this grant was erroneously reported on the original Form 3 filed June 03, 2019. The original filing reported the number of options as 34,891 options, but the actual grant was for 47,391 options.
(2)	The option vests on a 4-year schedule: 25% of the shares become exercisable on 5/2/2020; the remaining 75% of the shares vests at a rate of 1/36th per month over the next 36 months so that the option will be fully vested and exercisable on 5/2/2023.
